PI25 Features

2728 form updates

• Update the 2728 form that is used to collect information about Entitlement and Enrollment
• Received approval from OMB for the 2728 form updates as of 11/30/23
• ADO has begun prioritizing, planning on how to implement in the system
• At this time there is no firm implementation date
• Leadership suggested on holding off on the official announcement until the system is closer to being fully updated
• CMS will not put out an official announcement at this time because the system is not yet ready to begin collecting information
• EOCT is already working on training and announcements that will need to go out
• Everything that was on the website was approved; there were some changes with numbering and lettering
• Are there any plans to partner with the EDIs and user community as the form is being built/implemented for their input?
  • For example, being able to provide feedback on what fields should and should not be mandatory
  • EDI's attend the EQRS system demos but functionality at that point has already been built.  EDIs would like an opportunity to be part of the requirements phase and provide their feedback before the functionality has been built/completed
  • Porsche did acknowledge this has been an ongoing request from the EDIs
  • Porsche will bring this up at the EQRS SMT meeting to discuss with Senior Management Team

Medrec required yes/no

• Medication Reconciliation is a section within Patient > Manage Clinical of EQRS, where data is collected from the patient regarding Medication Reconciliation.  Right now, if data shows up in the fields, it is presumed the MedRec was performed.  If there is no data in the field, the assumption is that MedRec was not performed.  The program feels that we cannot tell from the current data whether the data input was missing/skipped/not entered when there is no information. The ask is to add a required Yes/No check box as the first question to help identify those that have vs have not done MedRec.

CDR ETL - Include Submitted Clinical Data for Open Months

• Make submitted clinical data for open months available to EQRS clinical dashboard report users and CDR data consumers so that users can view more recent clinical submissions and take action if needed before the clinical month closes. 

Vaccination Dashboard Reports for Flu

• Create a new Quicksight dashboard available within the EQRS application for accessing detailed vaccination data. 

Transplant Dashboard Enhancement - Add Inactive Reason Column and Update Removal Reasons Reference Table

• Add an inactive reasons column to the transplant dashboard for dialysis facilities so that social workers can see why their patients are inactive on the waitlist.
• When the transplant dashboards were being developed UNOS was not yet including the inactive reason codes in the files that are downloaded from the UNOS portal and processed to EQRS each week.  At the request of stakeholders, we have made a request of UNOS and gotten approval through HRSA to add the inactive reason code to the files downloaded for our ESRD patients.
• In addition, UNOS has begun using some new removal reason codes and we need to update the EQRS reference table with those codes so that the correct removal reason is displayed in EQRS instead of "Unknown".
• Will the NCC be able to consume the new data and disseminate it to the data recipients (EDIs and networks)?
  • Yes, the NCC will be able to consume the new data
  • It will be part of the transplant tables in EQRS
  • The NCC will be able to get from redshift and is also in CDR
  • Per Janet Lee (NCC) - the fact that is shows up in redshift tables, it's not going to be instantaneous that it shows up in their exports until there actually is data there and can actually process it to start putting it into those output tables
  • It will be a slightly iterative process once the data is actually there

DaVita - Not available on the call

FMC - No additional issues or updates

DCI - Is there any update to the Peritonitis?

• • There are no new updates except the frequency of the meetings has been increased and more parties are involved

RHA - Not available on the call

US Renal - No additional issues or updates

Innovative Renal Care - Is there a draft of the 2728 form available for them to look at?

• • There is a draft on the PRA website
  • All things were accepted.  There were some changes with numbering and lettering
  • Porsche will send the link to Laura Dabrowski
    • PRA website - all changes have been accepted for implementation with the addition of non-substantive changes (i.e. numbering and lettering)

Leah Skrien (network) offered to provide input/feedback on the updates to the 2728 form prior to the next Program monthly call.  In addition, assist in looking at what the EOCT team is coming up with regarding Q&A.

CMS will refresh data in the EQRS Prod Preview environment on the first Sunday of every month. The planned schedule is below.
