The CCSQ Data & Analytics Request Form serves to address and track requests from SAS Viya and CDR users. 

1. New or out of sync refresh data in the CDR.

2. Integration of analytic tools within CDR.

3. New functionality you would like in SAS Viya.

4. Workbench integration with CDR request.

Note that the is not a comprehensive list. 

Click the CCSQ Data & Analytics Request Form button at the top of the page to open the form, complete the required fields, provide as much detail as possible to avoid delays, and click Submit.

Note: the fields with an asterisk are required.

1. Summary, or short title of the request.

2. Specify: 

   1. Describe the business requirements (who, what, why, where, when).

   2. Describe the problem you are trying to solve.

   3. Describe the business justification.

3. Any files for reference.

4. Need by date. 

5. Submitter email.

6. Name of submitter.

Your request will be placed in a queue for response by the CCSQ Data & Analytics team. The team will contact you upon receipt with the status of your request.
