1. Log into ServiceNow (SNOW) Portal with your HARP ID.
If you have trouble logging in, please contact the QualityNet help desk.
2. On the bottom center of the page, scroll down and click “Create Incident” to create a new security incident ticket.
3. At this point you will be able to fill in the information relating to your report. The fields with a red asterisk will be required to be filled out.
PLEASE DO NOT INCLUDE ANY SENSITIVE INFORMATION WITHIN THE TICKET. THESE WOULD BE CONSIDERED VIOLATIONS.
The fields required include:
Reported by Email – Your email address
Reported by First Name – Your First Name
Report by Last Name – Your Last Name
Office Number – Best number to reach you at (can be same as Cell)
Cell Number – Best number to reach you at (can be same as Office)
Date/Time of Activity First Detected – When was the incident first noticed
Date/Time of Initial Report – Current date and time of you filling out the report
Organization – This should pre-populate but if not, enter your ADO organization
How was this incident detected/discovered? – A short description of the incident and how it was discovered.
4. Once the ticket is filled out and have your information ready to go (it would be best to type in a notepad or other stand-alone application in case the system freezes or errors out), hitSUBMITand the ticket then would go to the HCQIS SOC team for investigation.
