CCSQ Data and Analytics Request Form

This page provides access to the CCSQ Data & Analytics Request Form, which serves to address and track requests from SAS Viya and Centralized Data Repository (CDR) users.

Warning

You must be an authenticated Confluence user to open and submit the CCSQ Data & Analytics request form. For organizations who do not have Atlassian licenses, please contact your COR to submit the request on your team’s behalf.

titleWhat is the purpose of the request form?
The CCSQ Data & Analytics Request Form serves to address and track requests from SAS Viya and CDR users.

titleWhen should I use the request form (Note: this is not a comprehensive list)?
  1. New or out of sync refresh data in the CDR.
  2. Integration of analytic tools within CDR.
  3. New functionality you would like in SAS Viya.
  4. Workbench or FileCloud integration Workbench integration with CDR request.

titleHow do I submit a request?
Please click Click the CCSQ Data & Analytics Request Form button below (in blue) to at the top of the page to open the form, complete the required fields (marked with a red asterisk), providing , provide as much detail as possible to avoid delays, and click SUBMIT Submit.
Note: the fields with an asterisk are required.

  1. Summary, or short title of the request.
  2. Please specifySpecify: 
    1. Describe the business requirements (Whowho, Whatwhat, Whywhy, Wherewhere, Whenwhen).
    2. Describe the problem you are trying to solve.
    3. Describe the business justification.
  3. Any files for reference.
  4. Need by date date. 
  5. Submitter Emailemail.
  6. Name of Submittersubmitter.

titleWhat happens after I submit the request form?
Your request will be placed in a queue for response by the CCSQ Data & Analytics team. The team will contact you upon receipt with the status of your request.

titleDo I still need to submit ServiceNow tickets?
Yes, if you are experiencing issues or have any questions within or about SAS Viya and/or data in CDR, please submit a ServiceNow ticket instead of using the CCSQ Data & Analytics Request Form. For See the instructions on how to submit a ServiceNow ticket, please click on this link.
