What are Forms


Forms allows you to enter information into a custom screen for processing by the systems.  They are created by the UFM Team and contain fields unique to the form. 


Why are Forms Used?


Forms are used by individuals to upload files

Form Options


There are three options to select under Forms: Available, Drafts and Submitted.


Available Forms

  • Available Forms will provide you a list of forms that you have access to and will provide a description of when the form is utilized.  If there are no forms available to select this may mean that your team/organization doesn't utilize this feature.  

Draft Forms

  • Drafts will contain any forms that were not completed and sent fully when accessed under Available Forms.

Submitted Forms

  • Submitted will show you any forms that you have submitted.
